Mechanism & Pharmacokinetics ▼
Mechanism: Competitive muscarinic receptor antagonist. Reduces involuntary detrusor muscle contractions by inhibiting acetylcholine effects in the bladder.
Absorption: Well absorbed orally. Food has minimal effect.
Metabolism: Hepatic via CYP2D6 and CYP3A4 to active metabolite 5-hydroxymethyl tolterodine.
Elimination: Renal and fecal. Half-life ~2–4 hours (longer in poor metabolizers).
Dosage & Administration ▼
Immediate-release (IR): 2 mg orally twice daily
Extended-release (ER): 4 mg orally once daily
Take with or without food
Dose adjustment needed in hepatic impairment, severe renal impairment, or when used with potent CYP3A4 inhibitors: 1 mg IR twice daily or 2 mg ER once daily
