Mechanism: Topiramate blocks voltage-dependent sodium channels, enhances GABA activity, antagonizes AMPA/kainate glutamate receptors, and inhibits carbonic anhydrase, reducing neuronal excitability.
Absorption: Rapid and nearly complete; bioavailability ~80%
Metabolism: Partially hepatic via CYP3A4 and CYP2C19; mainly excreted unchanged in urine
Half-life: ~21 hours (can vary with enzyme-inducing drugs)

Start low, titrate gradually to minimize side effects
Typical starting dose: 25 mg/day, increase weekly by 25–50 mg
Target dose: 100–200 mg/day for monotherapy; 200–400 mg/day for adjunct therapy
Take twice daily; can be taken with or without food
Adjust dose in renal impairment
