Mechanism & Pharmacokinetics ▼
Mechanism: Inhibits angiotensin-converting enzyme, reducing formation of angiotensin II, leading to vasodilation and decreased aldosterone secretion.
Cardiac Effect: Reduces preload and afterload, improves ventricular remodeling.
Pharmacokinetics: Prodrug converted to active ramiprilat; peak 2–4 hours; half-life 13–17 hours; primarily renal excretion.

Note: Take at the same time each day. Swallow whole with water. Monitor blood pressure, kidney function, and electrolytes regularly.
Side Effects ▼
Common: Dry cough, dizziness, hypotension
Occasional: Hyperkalemia, fatigue, headache
Rare: Angioedema, renal impairment, severe hypotension
Contraindications ▼
History of angioedema related to ACE inhibitors
Pregnancy (especially 2nd and 3rd trimester)
Bilateral renal artery stenosis
Drug Interaction ▼
Potassium supplements or potassium-sparing diuretics increase hyperkalemia risk
NSAIDs may reduce antihypertensive effect
Lithium toxicity risk increased

Pregnancy Category D; contraindicated especially in later trimesters
Use during breastfeeding only if clearly necessary
