Fosfomycin Sodium

Rx / Antibiotic
Generic NameFosfomycin Sodium
Therapeutic ClassPhosphonic Acid Derivative Antibiotic
RouteIV / Oral
Max / 24h12 g IV / 3 g Oral single dose (UTI)
PregnancyCategory B
Indications ▼
Uncomplicated urinary tract infections (UTIs) in adults and children
Complicated UTIs caused by susceptible Gram-positive and Gram-negative bacteria
Pyelonephritis
Prophylaxis of urinary tract infections in urological procedures
Other infections (off-label) caused by susceptible strains, including soft tissue infections and respiratory infections
Mechanism & Pharmacokinetics ▼
Mechanism: Inhibits bacterial cell wall synthesis by inactivating the enzyme UDP-N-acetylglucosamine enolpyruvyl transferase (MurA), preventing peptidoglycan formation.
Pharmacokinetics: Rapidly absorbed orally (bioavailability ~37%); IV administration achieves high plasma levels. Widely distributed, excreted largely unchanged via urine. Half-life ~2–3 hours. Dose adjustment required in severe renal impairment.
Dosage & Administration ▼
IndicationDoseFrequency
Uncomplicated UTI (adult, oral)3 gSingle dose
Complicated UTI / IV therapy4–6 gEvery 8–12 hours IV infusion
Severe infections (IV)8–12 gDivided doses q8–12h
Note: Administer IV slowly over 30–60 minutes. Oral granules should be dissolved in water before ingestion.
Side Effects ▼
Diarrhea, nausea, vomiting
Headache, dizziness
Injection site reactions (pain, phlebitis)
Rare: allergic reactions, hypokalemia
Contraindications ▼
Hypersensitivity to Fosfomycin or any excipients
Severe renal impairment without dose adjustment
Pediatrics under approved age for oral formulation
Drug Interaction ▼
Nephrotoxic drugs: may increase risk of kidney injury
Probenecid may reduce renal excretion of Fosfomycin
Live vaccines may have reduced efficacy when combined with antibiotics
Pregnancy & Lactation ▼
Category B: No evidence of risk in humans
Excreted in breast milk in small amounts; caution advised
Clinical / Research Summary ▼
Therapeutic Role: Broad-spectrum antibiotic effective against Gram-positive and Gram-negative bacteria, including multidrug-resistant strains.
Clinical Effectiveness: Rapid bactericidal action in UTIs; high urinary concentrations achieved after single dose oral therapy.
Safety Profile: Generally well tolerated; gastrointestinal effects most common; rare hypersensitivity reactions reported.
Current Research: Focus on efficacy in multidrug-resistant urinary pathogens, alternative dosing regimens, and combination therapy studies.
