Indications
This fixed-dose combination is indicated for steroid-responsive inflammatory ocular conditions where a superficial bacterial ocular infection exists or a risk of bacterial ocular infection is high:
  • Ocular Inflammation: Effective for conditions such as cyclitis, iritis, and various forms of conjunctivitis.
  • Post-Surgical Inflammation: Used to manage inflammation and prevent infection following cataract or refractive surgery.
  • Bacterial Blepharoconjunctivitis: Treatment of inflammation of the eyelids and conjunctiva caused by bacteria.
  • Corneal Injury: To prevent infection and reduce swelling from chemical, radiation, or thermal burns.
Presentation
The medication is typically available in the following ophthalmic formats:
  • Ophthalmic Suspension: Loteprednol Etabonate 0.5% and Tobramycin 0.3% sterile drops.
  • Ophthalmic Ointment: Sterile ointment base for nighttime use or sustained ocular contact.
Description & Pharmacokinetics
This combination pairs a "soft" corticosteroid with a potent aminoglycoside antibiotic.

Loteprednol Etabonate is a unique corticosteroid designed to be metabolically inactivated rapidly by esterases in the eye, which significantly reduces the risk of increasing intraocular pressure compared to traditional steroids like Prednisolone. It works by inhibiting the inflammatory response, including edema, fibrin deposition, and capillary dilation.

Tobramycin provides the antimicrobial component. It inhibits protein synthesis in bacteria by binding to the 30S ribosomal subunit. It is highly effective against common ocular pathogens, including Staphylococcus aureus and Pseudomonas aeruginosa.

Pharmacokinetically, both drugs act locally. Loteprednol has high lipid solubility to penetrate cells, while Tobramycin maintains high concentrations in the tear film and corneal tissue with minimal systemic absorption.
Extra Important Information: Usage Insights
Key clinical instructions for the patient:
  • Shake Well: The suspension must be shaken vigorously before each use to ensure the active ingredients are evenly distributed.
  • Avoid Tip Contamination: Do not let the dropper tip touch the eye, eyelid, or any other surface to prevent bacterial contamination of the bottle.
  • Contact Lenses: Do not wear contact lenses while the eyes are infected or inflamed. The preservative (Benzalkonium Chloride) can also be absorbed by soft lenses.
  • Tapering: Do not stop using the drops abruptly if used for more than a few days. The dose should be gradually reduced as advised by the ophthalmologist to prevent rebound inflammation.
Side Effects
Common side effects may include:
  • Local Irritation: Burning or stinging sensation upon instillation.
  • Vision Changes: Temporary blurred vision or light sensitivity (photophobia).
  • Increased IOP: Although lower risk with Loteprednol, long-term use can still lead to increased intraocular pressure.
  • Secondary Infection: Fungal or viral overgrowth may occur with prolonged steroid use.
  • Eyelid swelling or itching (chemosis).
Pregnancy & Lactation
  • Pregnancy: Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Systemic absorption is low but not zero.
  • Lactation: It is not known whether topical ocular administration results in sufficient systemic absorption to be detectable in human milk. Exercise caution when breastfeeding.
Precautions & Interactions
  • Viral Diseases: Contraindicated in most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis).
  • Fungal Infections: Should not be used in fungal diseases of ocular structures.
  • Monitoring: If used for 10 days or longer, intraocular pressure (IOP) should be monitored by an eye specialist.
  • Glaucoma: Use with extreme caution in patients with a history of glaucoma.
Storage
Store the bottle upright at room temperature (15°C to 25°C). Protect from freezing and direct light. Discard 28 days after first opening. Keep out of reach of children.
