Mechanism & Pharmacokinetics ▼

Mechanism: Terazosin selectively blocks postsynaptic alpha-1 adrenergic receptors in vascular smooth muscle and prostate/bladder neck. This leads to vasodilation (lowering blood pressure) and relaxation of bladder outlet (improving urine flow in BPH).

Pharmacokinetics: Well absorbed orally with bioavailability of ~90%. Metabolized in the liver. Half-life: 9–12 hours. Excreted via urine and feces.

Note: Start at low dose to minimize first-dose hypotension. Dose adjustment may be needed in hepatic impairment.

Hypersensitivity to Terazosin or other quinazoline derivatives

History of syncope related to first-dose alpha-blocker therapy

Drug Interaction ▼

Other antihypertensives: additive hypotensive effect

Sildenafil, tadalafil: increased risk of hypotension

NSAIDs: may reduce antihypertensive effect

Strong CYP3A4 inhibitors or inducers: potential altered metabolism

Breastfeeding: caution advised; small amounts may be excreted in milk
