Therapeutic Class: Combined Leukotriene Receptor Antagonist & Antihistamine
Indications
This combination is indicated for:
  • Allergic Rhinitis: Relief of symptoms associated with seasonal or perennial allergic rhinitis.
  • Comorbid Asthma & Allergy: Management of patients where both leukotriene antagonism and antihistamine action are required to control symptoms like sneezing, rhinorrhea, and airway inflammation.
Presentation
Commonly available in fixed-dose combination tablets:
  • Tablet: Montelukast 10 mg and Fexofenadine Hydrochloride 120 mg.
  • Tablet: Montelukast 10 mg and Fexofenadine Hydrochloride 180 mg.
Description
This combination therapy targets two different inflammatory pathways. **Montelukast** is a selective leukotriene receptor antagonist that inhibits the $CysLT_1$ receptor, reducing bronchoconstriction and mucosal edema. **Fexofenadine** is a second-generation, non-sedating H1-receptor antagonist that blocks the effects of histamine. Together, they provide more comprehensive relief than either drug used alone.
Side Effects
  • Gastrointestinal: Nausea, diarrhea, and dyspepsia.
  • Neurological: Headache, dizziness, and drowsiness.
  • Neuropsychiatric: Agitation, sleep disorders, or mood changes (related to Montelukast).
  • General: Fatigue and dry mouth.
Pregnancy & Lactation
  • Pregnancy: Should be used only if the potential benefit justifies the potential risk to the fetus. Category B for both components individually.
  • Lactation: Caution should be exercised. Fexofenadine is known to pass into breast milk. It is advised to consult a physician before use during breastfeeding.
Precautions & Interactions
  • Neuropsychiatric Events: Monitor for behavioral changes.
  • Antacids: Aluminum and magnesium-containing antacids reduce the absorption of Fexofenadine.
  • Fruit Juices: Grapefruit, orange, and apple juices may reduce the bioavailability of Fexofenadine.
  • Renal Impairment: Dose adjustment of Fexofenadine may be necessary for patients with decreased renal function.
Storage
Store in a cool and dry place, protected from light and moisture. Maintain a temperature below 30°C. Keep tablets in the original **Alu-Alu blister** to ensure the stability of both active ingredients.
