Indications
Estriol vaginal preparations are primarily used to treat symptoms caused by estrogen deficiency in the urogenital tract:

Description & Pharmacokinetics
Estriol is a natural female sex hormone. Unlike other estrogens, Estriol is short-acting because it has a short retention time in the nuclei of endometrial cells. When applied locally in the vagina, it restores the normal vaginal flora and the physiological pH, increases the resistance of the vaginal epithelial cells to infection and inflammation, and reduces physical discomfort.
Pharmacokinetically, local administration of Estriol ensures that the hormone is delivered directly to the target tissues. While some systemic absorption does occur, it is generally much lower than oral estrogen therapy, leading to a safer profile regarding the stimulation of the endometrium (uterine lining). It is rapidly eliminated from the body through the urine.

- Dose Titration: Treatment usually starts with a daily application for the first 2 to 3 weeks, followed by a maintenance dose of twice-weekly applications.
- Endometrial Safety: Because Estriol is short-acting and used locally at low doses, it typically does not require a progestogen to protect the uterus in women with an intact uterus, though long-term use should be monitored.
- Latex Interaction: Some vaginal creams may weaken latex products like condoms or diaphragms. Patients should be advised on alternative contraceptive methods if applicable.

Pregnancy & Lactation
- Pregnancy: Strictly contraindicated. There is no indication for the use of Estriol during pregnancy. If pregnancy occurs during treatment, Estriol should be withdrawn immediately.
- Lactation: Not recommended. Estriol is excreted in breast milk and may decrease the quality and quantity of milk production.

- Undiagnosed Bleeding: Must not be used in patients with undiagnosed vaginal bleeding.
- History of Cancer: Contraindicated in women with known or suspected estrogen-dependent tumors (e.g., breast or endometrial cancer).
- Thrombosis: Caution in patients with a history of deep vein thrombosis (DVT) or pulmonary embolism.
- Drug Interactions: While systemic interaction is low, drugs like Rifampicin or Phenobarbital might increase the metabolism of estrogens.
