Therapeutic ClassDirect-acting antiviral (DAA)
Common Strength400 mg tablet
PregnancyUse only if clearly needed
Indications ▼
Chronic hepatitis C virus (HCV) infection in adults and adolescents ≥12 years
Used in combination with other DAAs (e.g., Ledipasvir, Velpatasvir) for better efficacy
Effective for all HCV genotypes depending on combination therapy
Mechanism & Pharmacokinetics ▼
Mechanism: Nucleotide analog inhibitor of HCV NS5B RNA-dependent RNA polymerase. Incorporation into viral RNA leads to chain termination and inhibits viral replication.
Pharmacokinetics: Rapidly absorbed; peak plasma concentration in ~0.5–2 hours. Metabolized to active triphosphate GS-461203. Mainly eliminated via urine. Half-life of active metabolite ~27 hours.
Dosage & Administration ▼
Dosage should be determined based on combination therapy and genotype; always follow physician guidance.
Indication / CombinationDoseFrequency / Notes
Chronic HCV (with Ledipasvir) 400 mg Sofosbuvir + 90 mg Ledipasvir Once daily for 12 weeks (may vary by genotype and prior treatment)
Chronic HCV (with Velpatasvir) 400 mg Sofosbuvir + 100 mg Velpatasvir Once daily for 12 weeks
Side Effects ▼
Fatigue, headache, nausea
Insomnia, dizziness
Anemia when combined with ribavirin
Rare hypersensitivity reactions
Contraindications ▼
Hypersensitivity to sofosbuvir or any excipient
Concomitant use with strong P-gp inducers (may reduce efficacy)
Drug Interaction ▼
Amiodarone: risk of symptomatic bradycardia
Strong P-gp inducers (rifampin, St. John’s Wort): reduce plasma levels
Co-administration with other DAAs is standard; monitor liver function
Pregnancy & Lactation ▼
Use only if clearly needed; consult physician
Excreted in breast milk unknown; weigh risk vs. benefit
Clinical / Research Summary ▼
Clinical Role: Key direct-acting antiviral for HCV; highly effective in combination therapy.
Advantage: Pan-genotypic activity, oral administration, high cure rates.
Limitation: Must be used in combination for most patients; expensive in some regions.
Monitoring: Liver function, HCV RNA viral load, renal function if comorbidities exist.
Patient Counseling Points ▼
Take exactly as prescribed, usually once daily
Do not skip doses; complete full course for viral clearance
Report unusual fatigue, jaundice, or severe nausea
Consult physician before starting any new medications
Storage & Handling ▼
