Therapeutic ClassAminoglycoside Antibiotic
Common Dose15 mg/kg/day IV/IM in 2–3 divided doses
Max / 24h1.5 g/day (adults)
PregnancyCategory D – Risk vs. benefit
Indications ▼
Serious Gram-negative bacterial infections (e.g., Pseudomonas, E. coli, Klebsiella)
Severe sepsis and septicemia
Respiratory tract infections, urinary tract infections, intra-abdominal infections
Bone and joint infections, endocarditis (as part of combination therapy)
Mechanism & Pharmacokinetics ▼
Mechanism: Binds to 30S ribosomal subunit, inhibiting bacterial protein synthesis; bactericidal.
Pharmacokinetics: Poor oral absorption; given IV/IM. Distributed in extracellular fluid. Minimal metabolism. Eliminated primarily unchanged via kidneys. Half-life ~2–3 hrs (normal renal function).

Side Effects ▼
Common: Injection site reactions, mild nausea
Ototoxicity: Tinnitus, hearing loss (especially with prolonged use)
Nephrotoxicity: Increased serum creatinine, proteinuria
Rare: Neuromuscular blockade, hypersensitivity reactions
Contraindications ▼
Hypersensitivity to aminoglycosides
Pre-existing hearing impairment or renal dysfunction (use with caution)
Myasthenia gravis (risk of neuromuscular blockade)
Drug Interactions ▼
Loop diuretics: Increased risk of ototoxicity
Other nephrotoxic drugs: Increased risk of renal toxicity
Muscle relaxants: Enhanced neuromuscular blockade
Pregnancy & Lactation ▼
Category D – use only if benefit outweighs risk
Avoid during breastfeeding or monitor infant for toxicity
Clinical / Research Summary ▼
Effective against multi-drug resistant Gram-negative infections
Used as combination therapy in severe sepsis and endocarditis
Monitoring serum levels reduces risk of nephrotoxicity and ototoxicity
Patient Counseling Points ▼
Report hearing changes, dizziness, or ringing in ears immediately
Ensure adequate hydration to reduce kidney risk
Complete full course of therapy even if symptoms improve
Avoid concomitant nephrotoxic or ototoxic drugs if possible
Storage & Handling ▼
